The USA defends the President at almost all costs. That is against an assassins bullets, but also against health problems. There is also secondary attempts to defend the First Lady (or probably one day First Gentleman), but the President is the primary person who is defended.
We do not get the same standard of healthcare as the President. The First Lady does not get the same standard of healthcare as the President. If you think everyone is equal, you are delusional.
The "USA" does not defend the president at all costs, that's the secret service's job.
One can assume the "first family" has the same access to medical treatment the president does.
Does the VP receive the same treatments available to the president? Is it "second rate"?
Of course we don't get the same standard of care, we aren't the president of the U.S.of A.
